Individual Service Plan (ISP):   A complete written description of all covered behavioral health services and other informal supports that have been identified through the assessment process that will assist the person to meet his/her specified goals. The ISP is documented in the comprehensive clinical record and provided to all agencies involved in providing services identified on the ISP.
